Personal Care Home: Winnipegosis Personal Care Home provides 24-hour nursing care for 20 individuals including medical, nursing, pharmacy, dietary, housekeeping, laundry, maintenance, recreation and spiritual care. The goal is to provide these services in a friendly, home-like environment that value and welcomes the input, care and support of family members. Efforts are made to meet both cultural and spiritual needs, depending on the resources available.

Transitional Care Unit: The Winnipegosis & District Health Centre provides a 14-bed transitional care unit. Transitional care is the care of a patient who does not require 24/7 medical supervision by a physician but still requires some 24/7 nursing care. There are a variety of different circumstances where this type of care is appropriate: patients who are waiting placement in a person care home, respite care and patients who are recovering from an acute illness but no longer requires 24/7 medical care only available at acute care sites.

Facility Respite Care: A planned, temporary admission of an individual who requires up to 24 hours of care and supervision to a respite bed for a predetermined length of stay. Facility Respite Care is intended to support family/informal caregivers by providing an opportunity to be relieved temporarily of their care giving responsibilities, to alleviate/prevent stress to care givers, to temporarily reduce demands on Home Care resources and to help prevent premature long-term care placement for clients living in the community. Respite admissions are supported with the intent of the client returning home following the agreed upon length of stay.

Palliative Care: Palliative Care is about living life fully, to the very end of life, with dignity, comfort, care and support. Palliative Care Services are provided in the home and in Transitional Care and Personal Care Home facilities. These services include Palliative Care Drug Access Program, pain and symptom Management, Psychological Support, Bereavement Support, Camp Bridges, Professional, Volunteer, Public Education Support. Winnipegosis & District health Centre has one Palliative Care Room.

Spiritual Care Services: A volunteer spiritual are coordinator provides both the Winnipegosis Transitional Care Unit and the Personal Care Home clients, residents, families and staff with access to a variety of spiritual care services including regular church services, musical programs, individual counseling and access to Palliative Care/Pastoral Care volunteers.

Scheduled Outpatient Services: Clients can receive a variety of nurse managed medication administration and treatments provided through primary care order/directions for example wound care treatment, injections and intravenous medication treatment.

Inpatient/Resident Therapy Services: Clients and residents within WDHC have access to limited services on a referral basis to Occupational Therapy, Speech Therapy, Physiotherapy, and a Dietitian, as provided by Prairie Mountain Health.

Medical Clinic: Dr. Alison Carleton offers primary health services as a general practitioner within WDHC. Clinic hours are Monday to Friday 9am-430pm. Her office contact phone number is 656-4492. Dr. Carleton is also the primary care physician for the personal care home and transitional care unit of Winnipegosis and District Health Centre.

Telehealth: MB Telehealth provides Manitobans with access to health services through the use of technologies, to overcome the barriers of distance, time and expense. Through the use of secure networks, MB Telehealth provides tools that connect people to the information and services they require to manage their health and well-being closer to home. Winnipegosis and District health Centre has two telehealth units and various rooms wired to house the unit.

Diagnostic (Shared Health) -Provide limited lab (phlebotomy and EKG) services available in Winnipegosis but it is important to note that currently, due to human resource and equipment challenges, there are no x-ray services available. Lab services offered in Winnipegosis are Tuesday and Wednesday from 8:30 am- 2:30 pm.

Home Care Program (PMH): The Manitoba Home Care Program is a community-based program that provides home support in addition to existing supports and community resources to individuals of any age, who require health services or assistance with activities of daily living in order to remain in their own homes. The program provides comprehensive regular assessments to ensure safe and effective service and facilitates planning for Person Care Home placement when circumstances require a move to 24-hour care. The home care program also offer Direct Service nurses for clients to offer a variety of in-home services, for example wound care, ostomy care, and ABI assessments. Community Bathing Program: Community individuals can access the wheelchair accessible shower and/or century tub bath program through the Home Care program with assistance from facility or home care staff throughout the bath if needed. (This program is currently suspended related to COVID-19).

Public Health (PMH): Public Health nurses and staff work closely with individuals, families or communities to promote and protect optimal health and prevent disease and illness. Public Health Services include Family Health, Communicable Disease Prevention and Management, Youth Services, Injury Prevention and Emergency Preparedness. Seniors Mental Health (PMH): Prairie Mountain Health has a team of mental health professionals with special training and experience in working with elderly clients. These professionals include psychiatrist, psychologists, social workers, psychiatric nurses, occupational therapists and activity instructors who will travel to communities. Their work includes seeing clients for assessment and support in a variety of settings for mental health problems commonly associated with aging, which may include depression, anxiety, memory loss, confusion, challenging behaviors, elder abuse, questions of risk or safety and follow-up. Clients who are waiting placement for a personal care home and residents of WDHC are able to access mental health services through the mental health resource nurse on a referral basis.

Adult Mental Health (PMH): The Adult Community Mental Health program is available to those who are over 18 years of age and who have symptoms of a mental health concern. Services may include mental health promotion, learning about medications and illness, brief therapy, long-term therapy and helping get back in the community. Emergency Medical Services/Medical Transportation (Shared Health) Shared Health offers emergency medical services within Winnipegosis and area. Winnipegosis Transitional Care and Personal Care Home collaborate with Prairie Mountain Health to provide safe client emergency/inter-facility transportation through the EMS system. Winnipegosis and District Health Centre also collaborates with various community medical transport programs to assist clients to arrange for transportation to various appointments.

Community Para-medicine Pilot Project (Shared Health) – Emergency Medical Services is expanding services provided to individuals within the community through provision of direct access to primary care and as well providing a bridge between community health services and acute/emergency care. Paramedics can assist community individuals to access primary care services (physicians, nurse practitioners, community direct service nurses, home care), as well primary care providers can refer clients to be visited by a paramedic for a variety of reasons including wellness checks, medication compliance, home hazard assessments and others (blood glucose checks, blood pressure checks).

Heart Health to Chronic Disease Education Program and Outpatient Dietitian Services (PMH) This program has a nurse and dietician teams that travel to various communities throughout the region on a regular basis. They cover a variety of topics to help you learn more about healthy lifestyles and skills to assist you in managing your health condition(s). Group classes and individual (phone or virtual) appointments are offered.

Community Volunteer Income Tax Program (CVITP) In collaboration with Prairie Mountain Health, and approved by Canadian Revenue Agency, WDHC offers a FREE program to individuals who qualify (one person with income under $35,000, two persons with income under $45,000, three persons with income under $47,500 plus add $2,500 for each additional dependent). Individuals can drop off their income tax information to the Winnipegosis Health Centre Business Office- 230 Bridge St. Winnipegosis, MB Monday to Friday: 8:30 am – 4:30 pm between March 9 and November 30. This program is run by CRA trained community volunteers.

Transportation/Handivan Services: The Municipality of Mossey River offers Handivan services for the community for local and long distance travel in a wheelchair accessible van. Individuals can contact the Municipal office to book transportation at 204-656-4791.

Cancer Care Manitoba-Breast Check – WDHC partners with Cancer Care Manitoba to offer a location for the mobile bus/clinic to be on site to offer mammograms screening clinics within the community on a regular basis. Call 1-855-95-CHECK to book an appointment.
